Opt-ins not registering in Aweber & not redirecting to thank you page

Hello,
I've installed and everything looks great appearance-wise, except when a name and email address is entered in the form, aweber doesn't capture the lead and the page doesn't redirect to the thank-you page I designate in the form design.
Here is where the form is live and not working:

Ok, i've got it to accept info... but it's still not redirecting. What's really weird is that it's playing the AUDIO from the video that's on the page, but nothing displays Rob, any ideas why?
For anyone else with the same problem, here's how to get it to capture leads, as I deduced from other forum posts:
1) Copy Aweber code
2) Choose 'Others' for 'Opt In Form Type:'
3) ctrl->f to do a search... find 'input id' and change the entire 'awf_field' entry to say 'isu_name'
4) ctrl-> to do a search... find 'input class' and change the entire 'awf_field' entry to say 'isu_email'
5) Process and put javascript on the page you want.
But again.... this only gets the form to accept, it doesn't redirect.
Any way to do this?

Unfortunately it seems we cant see the content of the form here. I saw your comment where you remove the script tag but it is sitll messy.
Can you please upload it as a file instead?
Also, what's your email address for the ISU account ? We can speed that up and help to do modification for you

All right John. It seems that now I sort of know why it is not redirecting.
I go to your website (http://www.easyhorsetraining.com/141/how-to-canter-a-horse/) and do a view HTML source and only then I found out that you are using ISU1.0
ISU1.0 will not redirect you to the thank you page. It will take the opt-in code, and display the thank you message.
If you want to get user to redirect to thank you page, the only option is ISU2.0.
I see you have got the form working correctly by change it to "Other Opt-In" and get id="isu_name" and also id="Isu_email" into the respective field.
